How do I change the name and sex marker on my health records?
To change the name and sex markers on your health records, you just need to ask your regular health professional to update the National Health Index (“NHI”) with this information. The NHI is used by all health and disability professionals to identify people and link them with the right health records.
To update your name, you need proof of a legal name change. If you haven’t legally changed your name, you can still ask that they use the name you want them to use.
You don’t need to provide proof of your sex marker, and it doesn’t cost anything.
